ZAP channel DIDs stopped working
In the last 2 days, the DID I have associated with my incoming ZAP channel is not passed.
I get the following in my log: "Executing [s@from-pstn:1] Set("Zap/3-1", "__FROM_DID=s") " and the ring group associated with any DID/any CID rings.
Up until last week this feature was working fine.
I am using PBX in a Flash 1.2 and Asterisk 1.4 and FreePBX 2.5 rc3.

Please post a call trace of one of those calls coming in. Please make sure that you do a set verbose 5 before hand so that we get enough detail. But I'll bet money that your provider changed something and the DID is not coming in the same way it was last week. Yes they should not do that but they do.
I was wrong
I just noticed this in the instructions for ZAP Channel DIDs
You MUST assign the channel's context to from-zaptel for these settings to have effect. It will be a line that looks like:
context = from-zaptel
I had also upgraded PBX in a Flash due to a security fix and that context had changed to "from-pstn"
