DISA should use pinsets and change to autheticate

Description

DISA should be able to hook into the same pinsets that the pinset module supplies to oubound routes. And it should switch to authenticate in the generated dialplan (if not already - I can't recall now). And have the option of setting the pincode to the account code (authenticate 'a' option) so that the disa calls can be tracked in the cdr.

To the extent any of this applies to callback, it should be similar.
