Unknown keywords in queue config

Description

I created a queue and noticed in the logs 

Jan  4 10:10:19 WARNING[7471] app_queue.c: Unknown keyword in queue '291': 
rtone at line 10 of queues.conf
Jan  4 10:10:19 WARNING[7471] app_queue.c: Unknown keyword in queue '291': eventmemberstatusoff at line 18 of queues.conf

I am assuming that rtone=1 is ringtone instead of MOH, but shouldn't eventmemberstatusoff actually be eventmemberstatus?

eventmemberstatusoff originally fixed from #1118 in r2506

was actually a mistake in the samples, and should be eventmemberstatus digium bug asterisk patch

rtone was added for extensions_additional.conf, but the retrieve_queues_conf_from_mysql.pl script does not exclude it, and it gets put into queues_additional.conf

both are problems and should be fixed.
