Description

When manually assigning a second line appearance, via the "End Point Device List" by typing in the phone's MAC address, breaks the extension.

Steps to reproduce:

Via the "End Point Device List", enter a MAC address of a phone that is already assigned to an extension. Select line 2 for the line and select and extension that is not already assigned to a phone and hit "add".

1. The above procedure results in a blank page after hitting "add".

2. The extension assigned to the second line appearance, now results in a blank page when clicking on it via "extensions" in the FreePBX gui.
