Add Column Picker to CDR Reporting Module

Description

The CDR layout in FreePBX 2.10 is fantastic... but it left out the CallerID Name field that a lot of us depended upon from the old 2.9 reports. We built an interim patch (see below), but it would be nice to have a pick list of columns to display which could be saved as a default (perhaps with an option to change it on the fly if someone ticked a checkbox).

The column picker would be excellent. Also the "next page" button would be useful. Complaints: When sorting for example, by "source", you can not get the most recent calls to appear first. I tried the "newest first", "oldest first" no change. If you want to even see the newest calls you have to increase the "result limit" to some number that is greater than the number of calls in the period you have on the screen. (Because there is not a "next page" button). The default color of the report, all white screen with tiny black font, is hard to separate the lines and columns. The old report was much easier to read. I applied the above patch and look forward to the, improved version.
