Alert Info for Polycom incorrect for newer firmware.

Description

The addition of Polycom values in r14120 is incorrect. The value 'Alert-Info: info-Auto Answer' should be 'Alert-Info: info=Auto Answer' (It's an equal sign, not a dash).

I labeled this as a regression because the addition of the Polycom entry with incorrect info broke existing installs that were fine without it.
