internal callerid not being set to "Display Name" and defaulting to device on new extensions

Description

the callerid is now being set to device <extnum> instead of "Display Name" <extnum> all extensions in all technology types. This was working, I think something in the guielements / hooks fix broke it. I ran out of patience looking. I'll defer it to you (Rob) since you are a bit more familiar with that stuff and I think it may have occured when you fixed the other problems.

On some technologies it also seems like 'device' <extnum> isn't even being picked up properly by the system and resulting in other stuff breaking that depends on callerid (like speakname, wakeup call (even though that is trixbox), voicmail, checking, etc.) See #1411 and #1414 most certainly from this.

Is there a fix for SIP, this seems to be just for IAX?
